PrimaryOne Health is currently seeking a Behavioral Health Outreach Specialist for our Columbus health centers.

Essential Job Responsibilities:

  • Provides highest level of patient care as a bridge between patients with Behavioral Health needs and their Behavioral Health and medical provider.
  • Coordinates with primary care providers and behavioral health clinicians to facilitate planning for patient admission to appropriate level of care.
  • Tracks caseload of patients receiving MAT services both at PrimaryOne Health centers and at collaborating agencies.
  • Attends community meetings as assigned by Director of Behavioral Health and Social Work.
  • Provides case management, coordination, referral tracking, and patient education to ensure continuous and comprehensive care for patients on case load.
  • Follows caseload of patients receiving MAT services to ensure all treatment providers are informed about care being provided and that medical information is exchanged in compliance with all regulatory guidelines.
  • Maintains communication with patients on waitlist concerning treatment options.
  • Responsible for any scheduling/rescheduling of patient appointments with Behavioral Health clinicians and MAT treatment providers.
  • Problem solves patient concerns and communicates to behavioral health and MAT providers.
  • Attendance, promptness, professionalism, the ability to pay attention to detail, cooperativeness with co-workers and supervisors, and politeness to customers, vendors, and patients.
  • Other duties or special projects as assigned.

Successful candidates must possess a high school diploma or equivalent. Two years' experience working in an integrated primary care setting with complex patients.
