Manager Of Care Coordination

The Buckeye Ranch is seeking a Manager of Care Coordination to join our Care Management Entity (CME). The Manager of Care Coordination manages the day to day of the Care Management program and provides supervision to Care Coordination Supervisors to ensure the provision of care coordination, adherence to high fidelity wraparound model, and development of care coordination team members. The manager works in collaboration with team members to ensure coordinated care to children, youth, and families served, efficiency in providing services, compliance with regulatory bodies, and smooth collaboration between team members and network of providers. Ensures team members maintain a customer service focus for families, referral sources, all external customers, and internal team members. Provides leadership across the team and department and adheres to the Core values of The Buckeye Ranch, trauma-informed care, culturally responsive and strengths-based approach.

What you'll do:

  • Supervision of Care Coordinator Supervisors.
  • Maintain oversight of compliance with documentation standards in accordance with OAC 5160-59.03.2.
  • Collaborate with the Utilization Manager on case assignment and other areas of production.
  • Assure documentation meets regulatory standards.
  • Participate in standing management committees.
  • Review all internal and external audit performance and work with team to make necessary changes to improve performance.

Who you are:

  • You have a bachelor's degree in a human service-related field.
  • You preferably have an active LSW or LPC.
  • You have four years of relevant work experience.
  • You have an active driver's license and proof of auto insurance.
